10. Prefabricated Induced Expanded DP flap for resurfacing the neck and cheek regions

Postburn hypertrophic scars involving the neck and cheek of a42-year-old male patient were totally excised and successfully replaced with prefabricated induced expanded (PIE) deltopectoral flaps. At the first operation, the free forearm adipofascial flap was transferred to the subcutaneous plane of the deltopectoral region of the superior thyroid artery and then the tissue expander was placed immediately under the flap. After expanding the flap and the skin, the second operation was performed 3 months after the first operation. The hypertrophic scar was completely removed and replaced with the expanded flap (30 * 13 cm). The donor site was closed primarily. By combining the free forearm adipofascial flap as a vascular career and the tissue expander, a large and thin skin flap with superior color/texture match was obtained. Although complicated procedures are required, this method could be a useful option for the treatment of postburn scar in the head neck region.
